HOW IT WORKS
A survey about the use of funded services and unfunded supports by older teens and adults with diagnosed FASD

The Adult Leadership Cooperative (ALC) of FASD Changemakers are a group of adults who have FASD. We are conducting our third Lay of the Land Survey of older teens and adults with a diagnosis of FASD.
Both our previous surveys have been published.
YOU CAN TAKE THIS SURVEY IF YOU ARE 18 YEARS OR OLDER AND HAVE A DIAGNOSIS OF FASD.
In this survey, we want to know IF people with FASD are able to access and/or use both FUNDED services and UNFUNDED supports. This is another important topic no one has yet surveyed. We plan to change that.
The questions are really important. We want government, systems and services to hear directly from adults with FASD about what services are truly like for people with FASD – not people with any other disability. What works; what doesn’t work; what your experiences are. In every section of the survey, we have also left space and asked you to tell us what else you think about different things.
We want to use the information gathered in this survey to provide a baseline of information that can be a starting place for government, policy makers, systems, organizations and service providers to take a hard look at what IS provided, what is NOT provided, what needs to be provided….and more importantly, what do adults with FASD think about existing services IF they are able to access any.
We need your help to do this. You can participate in this survey no matter where you live in the world. This survey is on-line AND totally anonymous. Your ISP cannot be tracked. No one, including the ALC, will know who you are or what you tell us. We would like to know some basic information about you, like age, gender, diagnosis and where in the world you live, but nothing that can identify you as an individual.
As with our previous surveys, we ask that you answer all the questions that you can and ignore any you don’t know the answer to, or that you simply do not want to answer. There is NO right or wrong answers or way to fill it out. Do what works for you.
Because this survey is long, or if you need help with it, please ask someone you trust who knows you well, to help you complete it. It does NOT have to be done all at once. You can take a break and come back and finish it later. And…pass the link to everyone you know who has FASD. The more people who participate, the bigger the impact it has. Remember, this is a survey being done by adults with FASD….for adults with FASD!
